Your nursing forms, digitized
Up to 5 approved templates, each up to 3 pages, with one consolidated revision round. Mobile-browser entry, electronic signatures, office review and downloadable PDF records.
03 / ENABLE
A team ready to use it
16 named accounts, agreed role permissions and two 60-minute remote training sessions. One agency workspace with 20 GB of document storage and scoped ongoing support.
04 / OPTIONAL
Time and attendance
Clock-in/out timestamps, elapsed working time, authorized review and CSV timesheet export. Corrections require a reviewer and a recorded reason. GPS, payroll and payer/state EVV connections are excluded.

Alphard Technologies remains the software provider. Custom domains, native mobile apps and source-code ownership are outside this proposal.

What do we need from MedPlus?
Your logo and brand colors, blank nursing forms, staff list, role assignments and a designated approver. Please allow review within 5 business days. New requirements or delayed inputs may change the launch date.
What support and changes are included?
Hosting, routine updates and defect correction within scope, plus 1 hour/month of how-to or minor configuration support without rollover. Monday–Friday, 9 am–5 pm Eastern, excluding US federal holidays; target initial response within 1 business day. Extra accounts are $19.50/user/month, extra standard forms from $130, and approved additional work $97.50/hour. No unapproved add-on charges.
What is outside this package?
Pharmacy dispensing, medication ordering or dose calculations, infusion-device integration, billing/claims, payroll, EHR integrations, historical migration, custom analytics, SMS and other CareHub modules. This is a scheduling and documentation package, not a replacement for your pharmacy management system.
How are launch readiness and records handled?
Hosting, licensing, access controls, audit history, backup/restore and export arrangements are confirmed before signing. Applicable privacy/security review and required agreements must be completed before patient data is entered. MedPlus owns its data and clinical content; export, retention and deletion arrangements are confirmed in the service agreement. Full conditions are in the downloadable proposal.
